Referral 

Thank you for your interest in joining the Charlotte Clubhouse!

To begin, please complete the referral form (below) with your healthcare provider. Once we receive your referral, we will reach out to schedule your tour.

​

Tours are available on Tuesdays and Thursdays at 10 AM and 2 PM, lasting approximately 30 minutes.

​

Feel free to bring your therapist, practitioner, family member, or significant other along with you for the tour. Afterward, you will have the option to join the Charlotte Clubhouse, and we will schedule your first day and orientation if you choose to move forward.
